Common cold

The common cold is a viral infection of your upper respiratory tract — your nose and throat. A common cold is usually harmless, although it may not feel that way at the time. Most people recover from a common cold in about a week or two. If symptoms don't improve, see your doctor. Symptoms of a common cold usually appear about one to three days after exposure to a cold-causing virus. Signs and symptoms of a common cold may include:

  • Runny or stuffy nose
  • Itchy or sore throat
  • Cough
  • Congestion
  • Slight body aches or a mild headache
  • Sneezing
  • Watery eyes
  • Low-grade fever
  • Mild fatigue

When to see a doctor

Seek medical attention if you have:

  • Fever of 103 F (39.4 C) or higher
  • Fever accompanied by sweating, chills and a cough with colored phlegm
  • Significantly swollen glands
  • Severe sinus pain

A cold virus enters your body through your mouth, eyes or nose. The virus can spread through droplets in the air when someone who is sick coughs, sneezes or talks. But it also spreads by hand-to-hand contact with someone who has a cold or by sharing contaminated objects, such as utensils, towels, toys or telephones. If you touch your eyes, nose or mouth after such contact or exposure, you're likely to catch a cold.

Cold viruses are almost always present in the environment. But the following factors can increase your chances of getting a cold:

  • Age
  • Immunity
  • Time of year
  •  Acute ear infection (otitis media)
  •  Wheezing
  •  Sinusitis
  •  Other secondary infections including strep throat (streptococcal pharyngitis) and pneumonia.

What you can do

Get plenty of rest and drink lots of fluids. There's no cure for the common cold. Antibiotics are of no use against cold viruses. Over-the-counter (OTC) cold preparations won't cure a common cold or make it go away any sooner, and most have side effects. Here's a look at the pros and cons of some common cold remedies.

  • Pain relievers. For fever, sore throat and headache, many people turn to acetaminophen (Tylenol, others) or other mild pain relievers. Keep in mind that acetaminophen can cause liver damage, especially if taken frequently or in larger than recommended doses. 
  • Decongestant nasal sprays. Adults shouldn't use decongestant drops or sprays for more than a few days because prolonged use can cause chronic rebound inflammation of mucous membranes. 
  • Cough syrups. The Food and Drug Administration (FDA) and the American Academy of Pediatrics strongly recommend against giving OTC cough and cold medicines to children younger than age 2. Over-the-counter cough and cold medicines don't effectively treat the underlying cause of a child's cold, and won't cure a child's cold or make it go away any sooner. These medications also have potential side effects, including rapid heart rate and convulsions.

You may not be able to cure your common cold, but you can make yourself as comfortable as possible. These tips may help:

  • Drink lots of fluids. Water, juice, clear broth or warm lemon water are all good choices. They help replace fluids lost during mucus production or fever. Avoid alcohol and caffeine, which can cause dehydration, and cigarette smoke, which can aggravate your symptoms.
  • Try chicken soup. Generations of parents have spooned chicken soup into their sick children's mouths. Now scientists have put chicken soup to the test, discovering that it does seem to help relieve cold and flu symptoms in two ways. First, it acts as an anti-inflammatory by inhibiting the movement of neutrophils — immune system cells that help the body's response to inflammation. Second, it temporarily speeds up the movement of mucus through the nose, helping relieve congestion and limiting the time viruses are in contact with the nasal lining.
  • Get some rest. If possible, stay home from work or school if you have a fever or a bad cough or are drowsy after taking medications. This will give you a chance to rest as well as reduce the chances that you'll infect others. Wear a mask when you have a cold if you live or work with someone with a chronic disease or compromised immune system.
  • Adjust your room's temperature and humidity. Keep your room warm, but not overheated. If the air is dry, a cool-mist humidifier or vaporizer can moisten the air and help ease congestion and coughing. Be sure to keep the humidifier clean to prevent the growth of bacteria and molds.
  • Soothe your throat. A saltwater gargle — 1/4 to 1/2 teaspoon salt dissolved in an 8-ounce glass of warm water — can temporarily relieve a sore or scratchy throat.
  • Use saline nasal drops. To help relieve nasal congestion, try saline nasal drops. You can buy these drops over-the-counter, and they're effective, safe and nonirritating, even for children. 

No vaccine has been developed for the common cold, which can be caused by many different viruses. But you can take some common-sense precautions to slow the spread of cold viruses:

  • Wash your hands. Clean your hands thoroughly and often, and teach your children the importance of hand-washing.
  • Scrub your stuff. Keep kitchen and bathroom countertops clean, especially when someone in your family has a common cold. Wash children's toys periodically.
  • Use tissues. Always sneeze and cough into tissues. Discard used tissues right away, and then wash your hands carefully. Teach children to sneeze or cough into the bend of their elbow when they don't have a tissue. That way they cover their mouths without using their hands.
  • Don't share. Don't share drinking glasses or utensils with other family members. Use your own glass or disposable cups when you or someone else is sick. Label the cup or glass with the name of the person with the cold.
  • Steer clear of colds. Avoid close, prolonged contact with anyone who has a cold.

Sinus Infection (Sinusitis)

Sinusitis is an inflammation, or swelling, of the tissue lining the sinuses. Normally, sinuses are filled with air, but when sinuses become blocked and filled with fluid, germs (bacteria, viruses, and fungi) can grow and cause an infection. Conditions that can cause sinus blockage include the common cold, allergic rhinitis (swelling of the lining of the nose), nasal polyps (small growths in the lining of the nose), or a deviated septum (a shift in the nasal cavity).

Who Gets Sinusitis?

About 37 million Americans suffer from at least one episode of sinusitis each year. People who have the following conditions have a higher risk of sinusitis:

  • Nasal mucous membrane swelling as from a common cold
  • Blockage of drainage ducts
  • Structural differences that narrow the drainage ducts
  • Nasal polyps
  • Conditions that result in an increased risk of infection such as immune deficiencies or taking medications that suppress the immune system.

In adults, the contributing factors are most frequently infections and smoking.

What Are the Signs and Symptoms of Acute Sinusitis?

Some of the primary symptoms of acute sinusitis include:

  • Facial pain/pressure
  • Nasal stuffiness
  • Nasal discharge
  • Loss of smell
  • Cough/congestion
  • Additional symptoms may include:
  • Fever
  • Bad breath
  • Fatigue
  • Dental pain

Acute sinusitis may be diagnosed when a person has two or more symptoms and/or the presence of thick, green, or yellow nasal discharge.

What Are the Signs and Symptoms of Chronic Sinusitis?

People with chronic sinusitis may have the following symptoms for 8 weeks or more:

  • Facial congestion/fullness
  • A nasal obstruction/blockage
  • Pus in the nasal cavity
  • Fever
  • Nasal discharge/discolored postnasal drainage

Additional symptoms of chronic sinusitis may include:

  • Headaches
  • Bad breath
  • Fatigue
  • Dental pain

How Is Sinusitis Diagnosed?

To diagnose sinusitis, your doctor will review your symptoms and give you a physical examination. The exam may include the doctor feeling and pressing your sinuses for tenderness. He or she may also tap your teeth to see if you have an inflamed paranasal sinus.Other diagnostic tests to assess the potential causes for sinusitis may include a mucus culture, nasal endoscopy (see below), X-rays, allergy testing, CT scan of the sinuses, or blood work.

How Is Sinusitis Treated?

Treatment for sinusitis depends on its severity.

Acute sinusitis. If you have a simple sinus infection, your health care provider may recommend treatment with decongestants like Sudafed and steam inhalations alone. Use of nonprescription decongestant nasal drops or sprays may also be effective in controlling symptoms. However, these medicines should not be used beyond their recommended use, usually four to five days, or they may actually increase congestion. If antibiotics are given, they are usually given for 10 to 14 days. With treatment, the symptoms usually disappear and antibiotics are no longer required.

Chronic sinusitis. Warm moist air may alleviate sinus congestion. A vaporizer or inhaling steam from a pan of boiling water (removed from heat) may also help. Warm compresses are useful to relieve pain in the nose and sinuses. Saline nose drops are also safe for home use. Use of nonprescription decongestant nasal drops or sprays might be effective in controlling symptoms, however, they should not be used beyond their recommended use. Antibiotics or oral steroids may also be prescribed.

What Happens If Sinusitis Is Not Treated?

Delaying treatment for sinusitis may result in suffering from unnecessary pain and discomfort. In extremely rare circumstances, untreated sinusitis can lead to meningitis or brain abscess and infection of the bone.

Influenza (Flu)

Influenza is a viral infection that attacks your respiratory system — your nose, throat and lungs. Influenza, commonly called the flu, is not the same as stomach "flu" viruses that cause diarrhea and vomiting. For most people, influenza resolves on its own, but sometimes, influenza and its complications can be deadly. People at higher risk of developing flu complications include:

  • Young children under 5, and especially those under 2 years
  • Adults older than 65
  • Residents of nursing homes and other long-term care facilities
  • Pregnant women
  • People with weakened immune systems
  • People who have chronic illnesses, such as asthma, heart disease, kidney disease and diabetes
  • People who are very obese, with a body mass index (BMI) of 40 or higher

Your best defense against influenza is to receive an annual vaccination. Initially, the flu may seem like a common cold with a runny nose, sneezing and sore throat. But colds usually develop slowly, whereas the flu tends to come on suddenly. And although a cold can be a nuisance, you usually feel much worse with the flu.

Common signs and symptoms of the flu include:

  • Fever over 100 F (38 C)
  • Aching muscles, especially in your back, arms and legs
  • Chills and sweats
  • Headache
  • Dry, persistent cough
  • Fatigue and weakness
  • Nasal congestion
  • Sore throat

When to see a doctor

Most people who get the flu can treat themselves at home and often don't need to see a doctor. If you have flu symptoms and are at risk of complications, see your doctor right away. Taking antiviral drugs within the first 48 hours after you first notice symptoms may reduce the length of your illness and help prevent more-serious problems.

Flu viruses travel through the air in droplets when someone with the infection coughs, sneezes or talks. You can inhale the droplets directly, or you can pick up the germs from an object — such as a telephone or computer keyboard — and then transfer them to your eyes, nose or mouth.

People with the virus are likely contagious from the day or so before symptoms first appear until about five days after symptoms begin, though sometimes people are contagious for as long as 10 days after symptoms appear. Children and people with weakened immune systems may be contagious for a slightly longer time.

Influenza viruses are constantly changing, with new strains appearing regularly. If you've had influenza in the past, your body has already made antibodies to fight that particular strain of the virus. If future influenza viruses are similar to those you've encountered before, either by having the disease or by vaccination, those antibodies may prevent infection or lessen its severity.

But antibodies against flu viruses you've encountered in the past can't protect you from new influenza subtypes that can be very different immunologically from what you had before.

Factors that may increase your risk of developing influenza or its complications include:

  • Age
  • Living conditions
  • Weakened immune system
  • Chronic illnesses
  • Pregnancy
  • Obesity

Usually, you'll need nothing more than bed rest and plenty of fluids to treat the flu. But in some cases, your doctor may prescribe an antiviral medication, such as oseltamivir (Tamiflu) or zanamivir (Relenza). If taken soon after you notice symptoms, these drugs may shorten your illness by a day or so and help prevent serious complications.

If you do come down with the flu, these measures may help ease your symptoms:

  • Drink plenty of liquids. Choose water, juice and warm soups to prevent dehydration.
  • Rest. Get more sleep to help your immune system fight infection.
  • Consider pain relievers. Use an over-the-counter pain reliever, such as acetaminophen (Tylenol, others) or ibuprofen (Advil, Motrin IB, others), to combat the achiness associated with influenza. Don't give aspirin to children or teens because of the risk of Reye's syndrome, a rare, but potentially fatal disease.

The Centers for Disease Control and Prevention recommends annual flu vaccination for everyone over the age of 6 months. Each year's seasonal flu vaccine contains protection from the three or four influenza viruses that are expected to be the most common during that year's flu season. The vaccine is typically available as an injection or as a nasal spray.

Controlling the spread of infection

The influenza vaccine isn't 100 percent effective, so it's also important to take measures such as these to reduce the spread of infection:

  • Wash your hands. Thorough and frequent hand-washing is an effective way to prevent many common infections. Or use alcohol-based hand sanitizers if soap and water aren't readily available.
  • Contain your coughs and sneezes. Cover your mouth and nose when you sneeze or cough. To avoid contaminating your hands, cough or sneeze into a tissue or into the inner crook of your elbow.
  • Avoid crowds. Flu spreads easily wherever people congregate — in child care centers, schools, office buildings, auditoriums and public transportation. By avoiding crowds during peak flu season, you reduce your chances of infection. And, if you're sick, stay home for at least 24 hours after your fever subsides so that you lessen your chance of infecting others.

Strep Throat

 

Strep throat is a common type of sore throat in children, but it's not very common in adults. Healthcare professionals can do a quick test to determine if a sore throat is strep throat and decide if antibiotics are needed. Proper treatment can help you feel better faster and prevent spreading it to others! Many things can cause that unpleasant, scratchy, and sometimes painful condition known as a sore throat. Viruses, bacteria, allergens, environmental irritants (such as cigarette smoke), chronic postnasal drip, and fungi can all cause a sore throat. While many sore throats will get better without treatment, some throat infections—including strep throat—may need antibiotic treatment.

How You Get Strep Throat

Strep throat is an infection in the throat and tonsils caused by group A Streptococcus bacteria (called "group A strep"). Group A strep bacteria can also live in a person's nose and throat without causing illness. The bacteria are spread through contact with droplets after an infected person coughs or sneezes. If you touch your mouth, nose, or eyes after touching something that has these droplets on it, you may become ill. If you drink from the same glass or eat from the same plate as the infected person, you could also become ill. It is also possible to get strep throat from contact with sores from group A strep skin infections.

Common Symptoms of Strep Throat

The most common symptoms of strep throat include:

  • Sore throat, usually starts quickly and can cause severe pain when swallowing
  • A fever (101°F or above)
  • Red and swollen tonsils, sometimes with white patches or streaks of pus
  • Tiny, red spots (petechiae) on the roof of the mouth  (the soft or hard palate)
  • Headache, nausea, or vomiting
  • Swollen lymph nodes in the front of the neck
  • Sandpaper-like rash

A Simple Test Gives Fast Results

Healthcare professionals can test for strep by swabbing the throat to quickly see if group A strep bacteria are causing a sore throat. A strep test is needed to tell if you have strep throat; just looking at your throat is not enough to make a diagnosis. If the test is positive, your healthcare professional can prescribe antibiotics. If the strep test is negative, but your clinician still strongly suspects you have this infection, then they can take a throat culture swab to test for the bacteria, but those results will take a little longer to come back.

Antibiotics Get You Well Fast

The strep test results will help your healthcare professional decide if you need antibiotics, which can:

  • Decrease the length of time you're sick
  • Reduce your symptoms
  • Help prevent the spread of infection to friends and family members
  • Prevent more serious complications, such as tonsil and sinus infections, and acute rheumatic fever (a rare inflammatory disease that can affect the heart, joints, skin, and brain)

You should start feeling better in just a day or two after starting antibiotics. Call your healthcare professional if you don't feel better after taking antibiotics for 48 hours. People with strep throat should stay home from work, school, or daycare until they have taken antibiotics for at least 24 hours so they don't spread the infection to others.

Be sure to finish the entire prescription, even when you start feeling better, unless your healthcare professional tells you to stop taking the medicine. When you stop taking antibiotics early, you risk getting an infection later that is resistant to antibiotic treatment.

More Prevention Tips: Wash Those Hands

The best way to keep from getting strep throat is to wash your hands often and avoid sharing eating utensils, like forks or cups. It is especially important for anyone with a sore throat to wash their hands often and cover their mouth when coughing and sneezing. There is no vaccine to prevent strep throat.

Pinkeye (Conjunctivitis)

Pinkeye (also called conjunctivitis) is redness and swelling of the conjunctiva, the mucous membrane that lines the eyelid and eye surface. The lining of the eye is usually clear. If irritation or infection occurs, the lining becomes red and swollen. See pictures of a normal eye and an eye with conjunctivitis .

Pinkeye is very common. It usually is not serious and goes away in 7 to 10 days without medical treatment.

Most cases of pinkeye are caused by:

  • Infections caused by viruses or bacteria.
  • Dry eyes from lack of tears or exposure to wind and sun.
  • Chemicals, fumes, or smoke (chemical conjunctivitis).
  • Allergies.

Viral and bacterial pinkeye are contagious and spread very easily. Since most pinkeye is caused by viruses for which there is usually no medical treatment, preventing its spread is important. Poor hand-washing is the main cause of the spread of pinkeye. Sharing an object, such as a washcloth or towel, with a person who has pinkeye can spread the infection. 

Pinkeye may be more serious if you:

  • Have a condition that decreases your body's ability to fight infection (impaired immune system).
  • Have vision in only one eye.
  • Wear contact lenses.

If the pinkeye is caused by a virus, the person can usually return to day care, school, or work when symptoms begin to improve, typically in 3 to 5 days. Medicines are not usually used to treat viral pinkeye, so it is important to prevent the spread of the infection. Pinkeye caused by a herpes virus, which is rare, can be treated with an antiviral medicine. Home treatment of viral pinkeye symptoms can help you feel more comfortable while the infection goes away.

An infection may develop when bacteria enter the eye or the area around the eye. Some common infections that cause pinkeye include:

  • Staph infection.
  • Haemophilus influenzae Haemophilus influenzae type b (Hib).
  • Cat-scratch disease.
  • Gonorrhea.

Symptoms of bacterial pinkeye include:

  • Redness in the white of the eye.
  • Gray or yellow drainage from the eye. This drainage may cause the eyelashes to stick together.
  • Mild pain.
  • Swelling of the upper eyelid, which may make the lid appear to droop (pseudoptosis).

Bacterial pinkeye may cause more drainage than viral pinkeye. Bacterial infections usually last 7 to 10 days without antibiotic treatment and 2 to 4 days with antibiotic treatment. The person can usually return to day care, school, or work 24 hours after an antibiotic has been started if symptoms have improved. Prescription antibiotic treatment usually kills the bacteria that cause pinkeye.

Mononucleosis (Mono)

Mononucleosis, also called "mono," is a common illness that can leave you feeling tired and weak for weeks or months. Mono goes away on its own, but lots of rest and good self-care can help you feel better. Mono usually is caused by the Epstein-Barr virus (EBV). It is most often seen in teens and young adults. Children can get the virus, but it often goes unnoticed because their symptoms are mild. Older adults usually don't get mono, because they have immunity to the virus.

Mono can be spread through contact with saliva, mucus from the nose and throat, and sometimes tears. Because the virus can be spread through kissing, it has earned the nickname the "kissing disease." If you have mono, you can avoid passing the virus to others by not kissing anyone and by not sharing things like drinking glasses, eating utensils, or toothbrushes.

As soon as you get over mono, your symptoms will go away for good, but you will always carry the virus that caused it. The virus may become active from time to time without causing any symptoms. When the virus is active, it can be spread to others. Almost everyone has been infected with the mono virus by adulthood.

The most common symptoms of mono are:

  • high fever
  • severe sore throat
  • swollen lymph nodes(sometimes called swollen glands) and tonsils
  • weakness and fatigue.

Symptoms usually start 4 to 6 weeks after you are exposed to the virus.

Mono can cause the spleen to swell. Severe pain in the upper left part of your belly may mean that your spleen has burst. This is an emergency. Your doctor will ask you questions about your symptoms and will examine you. You may also need blood tests to check for signs of mono. Blood tests can also help rule out other causes of your symptoms.

Usually only self-care is needed for mono.

  • Get plenty of rest. You may need bed rest, which could keep you away from school or work for a little while.
  • Gargle with salt water or use throat lozenges to soothe your sore throat. This is okay for children as long as they are old enough.
  • Take acetaminophen (such as Tylenol) or ibuprofen (such as Advil) to reduce fever and relieve a sore throat and headaches. Never give aspirin to someone younger than age 20 years, because it can cause Reye syndrome, a serious illness. Be safe with medicines. Read and follow all instructions on the label.
  • Avoid contact sports and heavy lifting. Your spleen may be enlarged, and an impact or straining could cause it to burst. In severe cases, medicines called corticosteroids may be used to reduce swelling of the throat, tonsils, or spleen.

Norovirus (Stomach Bug)

Nothing can ruin a vacation like a bout of vomiting, diarrhea, and stomach cramps. Noroviruses have become notorious for sending hundreds of cruise ship passengers at a time running for their respective bathrooms and for steering entire ships back to port early. Back on dry land, noroviruses also have a big impact on people's health. The CDC estimates that noroviruses are responsible for more than half of all food-borne disease outbreaks each year. And noroviruses are the most common cause of diarrhea in adults and children.

What Are Noroviruses?

Noroviruses are a group of viruses that cause inflammation of the stomach and large intestine lining (gastroenteritis); they are the leading cause of gastroenteritis in the U.S. The norovirus was originally called the Norwalk virus after the town of Norwalk, Ohio, the location of the first confirmed outbreak in 1972.

Noroviruses are sometimes called food poisoning, because they can be transmitted through food that's been contaminated with the virus. They aren't always the result of food contamination, though. Noroviruses are also sometimes called the stomach flu, although they aren't the influenza virus.

What Causes Infection With Noroviruses?

People become infected with noroviruses when they eat food or drink liquids that have been contaminated; raw or undercooked oysters and raw fruits and vegetables have been implicated in some outbreaks. You can also get infected if you touch an object or surface that has been infected with the virus and then touch your nose, mouth, or eyes.

Noroviruses thrive on cruise ships (as well as in daycare centers, restaurants, nursing homes, and other close quarters), because they are very hardy and highly contagious. They can survive temperature extremes in water and on surfaces. Once someone is infected from contaminated food, the virus can quickly pass from person to person through shared food or utensils, by shaking hands or through other close contact. People who have a weakened immune system are particularly susceptible to catching noroviruses.

What Are the Symptoms of a Norovirus Infection?

If you come down with a norovirus infection, you'll probably go from being completely healthy to feeling absolutely miserable within a day or two after being exposed to the virus. Typical symptoms include nausea, vomiting (more often in children), watery diarrhea (more often in adults), and stomach cramps.

Other norovirus symptoms include:

  • Low-grade fever
  • Chills
  • Headache
  • Muscle aches
  • Fatigue

Most of these symptoms aren't serious, but diarrhea and vomiting can deplete your body of the fluid it needs and you can become dehydrated. Children and the elderly are most susceptible to dehydration, as well as malnutrition from not getting enough nutrients. If you have symptoms of norovirus, your doctor can give you a stool test to confirm that you have the illness. However, norovirus diagnosis is usually made based solely on symptoms.

Norovirus Treatment

Noroviruses, like other viruses, don't respond to antibiotics, which are designed to kill bacteria. No antiviral drug can treat noroviruses, but in healthy people the illness should go away on its own within a couple of days. Most people don't have any long-term problems from the virus. To prevent dehydration, make sure to drink plenty of liquids, especially water and juices. Avoid sugary drinks, which can make diarrhea worse, as well as alcohol and caffeinated beverages, which can dehydrate you further.

How to Prevent Norovirus Infection

Good hygiene is the key to preventing an infection with norovirus, especially when you are in close surroundings with a lot of other people. 

  • Wash your hands often with soap and water for at least 15 seconds, or use an alcohol-based hand sanitizer, especially after going to the bathroom or changing a baby's diaper, and before you prepare or eat food.
  • Carefully dispose of any contaminated items (such as dirty diapers).
  • Wash raw fruits and vegetables thoroughly. Cook oysters and other shellfish before eating them.
  • Clean and disinfect surfaces with a mixture of detergent and chlorine bleach after someone is sick.

If you have norovirus, don't prepare food for at least two to three days after you feel better. Try not to eat food that has been prepared by someone else who is sick.
