Academic Performance Concern (Section 3)

We want to be proactive in connecting students to key people and resources when academic concerns  are present. When professors observe a student neglecting course work, interfering with the discipline of the course, or not attending class, they can submit an Academic Performance Concern (also known as a Section 3) form. The purpose of this process is to inform the student, their academic advisor, and academic support staff of the concern so a plan of action can be created and support and resources can be offered. 

Once students receive the electronic letter, they are required to meet with their professor and advisor to create a plan to be successful in the class, then fill out this DocuSign form for their professor and advisor to electronically sign. If the student does not follow this process or adjust their behavior, an instructor can submit an additional Academic Performance Concern (Section 3) requesting that the student be withdrawn from the course.
