Part I: Notice of Process - due three (3) weeks prior to the start of new member education/intake (PDF version to preview)
This form is to be completed by the New Member Educator/Intake Coordinator. Contact information for chapter's current President, Alumni Advisor and Inter/national Headquarters Staff is required.
Part II: Verification of Initiates - due two (2) business days following initiation (PDF version to preview)
This form is to be completed by the New Member Educator/Intake Coordinator. Contact information for chapter's current President and Alumni Advisor is required.
Part IV - The following forms are to be completed if appliable to your chapter
4.1 Additional New Member Education Plan - due one (1) week prior to the start of additional New Member Education (PDF version to preview)
This form is to be completed by the New Member Educator/Intake Coordinator. Contact information for chapter's current President and Alumni Advisor is required.
4.2 New Member Presentation Outline - due two (2) weeks prior to the New Member Presentation (PDF version to preview)
This form is to be completed by the New Member Educator/Intake Coordinator. Contact information for chapter's current President and Alumni Advisor is required.
Part III - New Member Drop Forms
New Member Drop Notice - Chapter Submission
This form is to be completed by the chapter within two (2) business days of decision to drop new member. Contact information for chapter's current President, New Member Educator, and Alumni Advisor is required.
New Member Drop Notice - New Member Submission
This form is to be completed by the student who is withdrawing from the New Member Education/Intake Process.
