New Member Education Forms Timeline
There are two forms required to be completed by the New Member Educator/ Intake Coordinator throughout the new member education and intake process.
Interfraternity Council and Panhellenic Council chapters must complete the Notice of Process form three (3) weeks prior to the start of their new member education process.
Cultural Greek Council chapters must complete the Notice of Process and New Member Presentation Outline form three (3) weeks prior to the start of their new member education process.
The Verification of Initiates form is to be completed within two (2) business days following the initiation of new members by all chapters.
Additional forms, such as the Additional New Member Education Plan and New Member Drop Notice, are only to be completed if applicable.

Notice of Process - due three (3) weeks prior to the start of new member education (PDF version to preview)
This form is to be completed by the New Member Educator. Contact information for the chapter's president, Alumni Advisor, and Inter/national Headquarters staff is required.
Notice of Process and New Member Presentation Outline - due three (3) weeks prior to the start of new member education (PDF version to preview) *CGC Organizations Only*
This form is to be completed by the New Member Educator/ Intake Coordinator. Contact information for the chapter's president, Alumni Advisor, and Inter/national Headquarters staff is required.
Verification of Initiates - due two (2) business days following initiation (PDF version to preview)
This form is to be completed by the New Member Educator/ Intake Coordinator. Contact information for the chapter's president, Alumni Advisor, and Inter/national Headquarters staff is required.
Additional New Member Education Plan - due one (1) week prior to the start of additional new member education (PDF version to preview)
This form is to be completed by the chapter's New Member Educator/ Intake Coordinator. Contact information for the chapter's president and Alumni Advisor is required.
New Member Drop Notice - Chapter Submission
This form is to be completed by the chapter within two (2) business days of the decision to drop the new member. Contact information for the chapter's president and Alumni Advisor is required.
New Member Drop Notice - New Member Submission
This form is to be completed by the student who is withdrawing from New Member Education/ Intake Process.
